Services

Clarity Assessments provides neurodevelopmental evaluations for children conducted by Dr. Shelby Wolf, PhD, LP, BCBA-D, NCSP. Our services include focused and comprehensive evaluations, intake sessions, testing, written reports, and feedback sessions.

All services are provided in accordance with applicable professional and ethical standards, including those set by the American Psychological Association and the Minnesota Board of Psychology.

Appointments & Cancellations

We require 48 hours notice for cancellations or rescheduling. Late cancellations or no-shows may be subject to a fee. Please contact us as soon as possible if you need to change your appointment.

We understand that unexpected circumstances arise. If you need to cancel with less than 48 hours notice due to an emergency or illness, please contact us and we will do our best to accommodate your situation.

Payment Terms

Payment is expected at the time of service unless prior arrangements have been made. We offer flexible payment plans and accept CareCredit financing. A deposit is required to secure your evaluation appointment.

We accept all major credit and debit cards, HSA/FSA cards, cash, and checks. All fees will be disclosed in your Good Faith Estimate prior to your evaluation.

Scope of Services

Clarity Assessments provides neurodevelopmental evaluations and written reports. Our services do not include ongoing therapy, treatment, or medical care. Evaluation results and recommendations should be discussed with your child's healthcare team and implemented in collaboration with appropriate professionals.

Our evaluations are conducted for diagnostic and educational planning purposes. They do not constitute medical diagnoses and should not replace consultation with a licensed physician or other medical professional.

Confidentiality

All information shared with Clarity Assessments is treated as confidential in accordance with HIPAA regulations and applicable state laws. We will not disclose your information to third parties without your written consent, except as required by law or as necessary to provide services.
